Our transportation services are available to eligible riders as determined by the applicable transportation broker and/or Medicaid program requirements. This website is intended for use by individuals who are at least 18 years of age.

- Submitting false or misleading information through any form on this website.

5. Employment Applications
By submitting an employment application or driver referral through this website, you represent that all information provided is accurate and complete. You authorize Serenity to verify the information provided and to conduct background checks, driving record reviews, and drug screenings as described in the application process.
6. Driver Referral Bonus Program
The Driver Referral Bonus Program is subject to the following terms:
- The referral bonus of $250 is payable only after the referred driver completes onboarding and a minimum of 25 trips within their first 60 days of active service.
- Self-referrals are not eligible for the bonus.
- Serenity reserves the right to modify or discontinue the referral program at any time with reasonable notice.
- Referral bonuses are subject to applicable tax withholding requirements.
- In the event of a dispute regarding referral eligibility, Serenity's determination shall be final.
